"As for Neeta since it seems to be a very severe form of osteoporosis, it will be prudent to follow the orthopod advise and get all the medications. Inj Aclasta( zolidronic acid) will be needed if her osteoporosis is severe since chances of brittle bones fracturing is very high. This is a very costly drug, so she should have very severe osteporosis with high risk of fractures to be advised this drug. Moreover it is extremely costly too!

What she could do is talk again to the orthopod and if ok start with calcium/VitD and Fosavance (which already contains alendronate, which is similar to zolidronic acid) and recheck after 6-8 months of this therapy by BMD and if no improvement then go for the injection. The injection to my knowledge is only once a year and may be given any time during the period of treatment. This according to me would be a more rational approach. However she must continue with walking and some mild exercise which has been proven to strengthen bones.
I have not seen her current BMD report, so better if she has a rechat with the orthopod and try the tablets route first and then injection if no improvement later."
